    • By registering for a Passaic County event you hereby authorize the County of Passaic to record, film, photograph or videotape your likeness, statements, performance, movement, work and/or experiences in any form.

    • Release of Liability & Waiver Form
    • This Release of Liability & Waiver Form must be completed and submitted to the Passaic County Parks & Recreation Office prior to the event start date & time. Please email this form to: Recreation@Passaiccountynj.org or fax it to: 973-872-2684.
